Asuhan Keperawatan Pada Pasien Fraktur Clavikula post Orif (Open Reduction Internal Fixation) Dengan Resiko Tinggi Infeksi Di RSUD Dr. Soetomo Surabaya

ABSTRAK
Fraktur clavikula adalah terputusnya hubungan tulang clavikula yang disebabkan oleh trauma langsung dan tidak langsung. Pada kebanyakan kasus fraktur, tindakan yang dapat dilakukan adalah terapi pembedahan dimana akan terjadi luka bekas dari tindakkan tersebut. Adanya luka tersebut apabila tidak dirawat dengan baik maka akan menyebabkan berbagai macam masalah salah satunya resiko tinggi infeksi. Tujuan dari penelitian ini untuk melakukan asuhan keperawatan pada pasien fraktur clavikula. Metode penelitian yang digunakan adalah studi kasus dengan menggunakan dua partisipan dari pasien yang dirawat selama 3 hari di RSUD dr. Soetomo Surabaya. Hasil studi menjelaskan bahwa pasien dengan fraktur clavikula didapatkan diagnosa keperawatan prioritas resiko infeksi. Intervensi yang diberikan pada dua pasien adalah jelaskan pada keluarga tentang tanda gejala dan pencegahan infeksi, ajarkan pada pasien dan keluarga tentang cara cuci tangan dengan benar, anjurkan ibu untuk meningkatkan nurtisi pasien, anjurkan ibu untuk menjaga kebersihan lingkungan dan kebersihan pasien, pertahankan teknik isolasi dengan batasi pengunjung, ganti IV line dengan teknik aseptic, kolaborasi dengan dokter dalam pemberian obat antibiotic, kolaborasi dengan petugas lab untuk mengobservasi leukosit, monitor tanda dan gejala infeksi. Dari rencana tersebut dilakukan tindakan sesuai rencana dan didapatkan evaluasi selama 3 hari masalah resiko infeksi tidak terjadi dan pasien pulang. Hal ini dikarenakan kedua pasien dapat menunjukkan perilaku hidup sehat dengan cara rajin mencuci tangan yang benar sesuai dengan yang diajarkan oleh perawat.

ABSTRACT
Fracture of the clavicle is the breaking of the clavicle bone connection caused by direct and indirect trauma. In most cases of fractures, the only action that can be taken is surgical treatment where there will be a scar from the action. The existence of these injuries if not treated properly will cause various kinds of problems one of them high risk of infection. The purpose of this study was to perform nursing care in patients with clavicle fractures. The research method used was case study using two participants from patients treated for 3 days in dr. Soetomo Surabaya. The results of the study suggest that patients with clavicle fractures have a priority risk diagnosis of nursing infection. Intervention given to two patients is to explain to the family about symptoms and prevention of infection, teach the patient and family about handwashing properly, encourage the mother to improve patient nurtition, encourage the mother to maintain the cleanliness of the environment and patient hygiene, maintain the isolation technique with Restrict visitors, replace IV lines with aseptic techniques, collaborate with doctors in antibiotic drug administration, collaboration with lab personnel to observe leukocytes, monitor signs and symptoms of infection. From the plan carried out action as planned and got evaluation for 3 days problem of infection risk did not happened and patient go home. This is because both patients can show healthy lifestyles by diligent hand washing properly in accordance with what is taught by nurses.
